Mallory Weiss Tear - An Uncommon Cause of Upper G.I Bleeding in a Cardiac Patient with Successful Dual Intervention

چکیده
Introduction: Primary Percutaneous Coronary Intervention (PPCI) with stenting, especially Drug eluting stents (DES), is the most widely accepted strategy globally for patients presented STEMI, which necessitates a long duration of Dual antiplatelet therapy (DAPT) to prevent stent thrombosis. Potent P2Y12 inhibitors DAPT are preferred in view their superior protection against thrombosis but at expense increased bleeding risks. Early therapeutic intervention event acute GI bleed has beneficial outcome while on active cardiac intervention.
Case Presentation: A 67-year-old patient, who underwent PPCI DES stent, incidentally developed Mallory-Weiss (MW) tear, manifested as severe hematemesis when loaded consisting Aspirin and Ticagrelor. The was withheld until clipping tear done. Single (SAPT) reinstituted after day plus Clopidogrel week.
Conclusion: careful assessment risks benefits coronary interventions need consider complications timely thus individualizing curating deemed necessary.

Mallory-Weiss sindrom uzrokuje 5-15% krvare...
متن کاملRisk Factors for an Iatrogenic Mallory-Weiss Tear Requiring Bleeding Control during a Screening Upper Endoscopy
Background and Aim. In some cases of iatrogenic Mallory-Weiss tears (MWTs), hemostasis is needed due to severe mucosal tearing with bleeding. Therefore, we aimed to evaluate the risk factors for severe iatrogenic MWTs and the methods of endoscopic bleeding control. Materials and Methods. Between January 2008 and December 2012, 426,085 cases of screening upper endoscopy were performed at the Asa...
متن کاملMallory-Weiss Tear during Esophagogastroduodenoscopy
Mallory-Weiss tears (MWTs) are mucosal lacerations caused by forceful retching and are typically located at the gastroesophageal junction. Reported cases of MWT with serious complications seen in esophagogastroduodenoscopy are limited. We report MWT in an 81-year-old woman who presented with gastric perforation by esophagogastroduodenoscopy. We discuss and indicate that hiatal hernia, atrophic ...
متن کاملMallory-Weiss Tear in the Duodenum
Mallory-Weiss tears are mucosal lacerations caused by forceful retching and are typically located at the gastroesophageal junction. Mallory-Weiss tears have not been described in the duodenum. We report of a Mallory-Weiss tear in the descending duodenum of a 57-year-old man who presented with hematemesis preceded by forceful retching. We discuss the pathophysiology of a duodenal injury in compa...
متن کاملOesophageal haematoma and associated Mallory-Weiss tear.
Intramural oesophageal haematoma is a rare condition that may present as vomiting or haematemesis. Mallory-Weiss tear has been proposed as a possible aetiology but the evidence to support this is circumstantial. A case of oesophageal haematoma associated with evidence of Mallory-Weiss tear on endoscopy that helps to support this hypothesis is presented.
